  • Follow the terms of the Notice currently in effect

  • Notify affected individuals following a breach of unsecured PHI when required by law

How We May Use and Disclose Your Health Information

We may use and disclose PHI to provide, coordinate, or manage behavioral healthcare and related services.

For example, we may share relevant information with other healthcare professionals involved in an individual's care when permitted by law.

Examples may include quality improvement, supervision, staff training, credentialing, auditing, compliance activities, business planning, and evaluating the quality of services.

Business Associates

We may disclose PHI to service providers who perform functions on our behalf that require access to PHI.

When required by HIPAA, these organizations are considered Business Associates and are contractually required to appropriately protect PHI.

Appointment and Service Communications

We may use your contact information to communicate about appointments, scheduling, treatment, service coordination, and other healthcare-related matters.

As Required or Permitted by Law

We may use or disclose PHI when required or permitted by applicable law, including circumstances involving public health activities, health oversight activities, judicial or administrative proceedings, law enforcement, or other legally authorized purposes.

Preventing Serious Threats to Health or Safety

When permitted by law, we may use or disclose PHI when reasonably necessary to prevent or lessen a serious and imminent threat to the health or safety of an individual or the public.

Abuse, Neglect, or Exploitation

We may disclose information when required or permitted by law concerning suspected abuse, neglect, exploitation, or other circumstances subject to mandatory reporting requirements.

Uses and Disclosures Requiring Authorization

Certain uses and disclosures of PHI require your written authorization unless otherwise permitted by law.

If you provide authorization, you may generally revoke that authorization in writing at any time. Revocation will not affect actions already taken in reliance on your authorization.

Wildflower ABA will not sell PHI or use PHI for purposes requiring authorization under HIPAA without obtaining the required authorization.
